MediAsas 2027: Can Employees Apply Directly? RM200 SME Subsidy & EPF Payments
MediAsas is due for public launch in January 2027, but the RM200 first-year SME employee premium subsidy is a separate issue. Learn whether workers can join individually, SME employee limits and EPF Sejahtera payment.

The short answer: MediAsas is not a health plan that can only be obtained through your boss. Bank Negara Malaysia (BNM) describes it as a voluntary medical and health insurance/takaful (MHIT) plan scheduled for national public rollout in January 2027. Members of the public can seek information from participating insurers or takaful operators when the plan launches.
But the RM200 government subsidy for SME employees' first-year premiums is a separate measure. Budget 2027 says it targets eligible SMEs with fewer than 75 employees, with RM200 per qualifying worker for the first year, capped at 50 employees per company.
As of 13 October 2026, the official announcements reviewed do not specify whether workers can claim the RM200 voucher individually or must apply through their employer. Do not confuse the general ability to obtain MediAsas with eligibility or the application channel for the SME subsidy.
MediAsas personal enrolment vs SME RM200 voucher
| Question | General MediAsas enrolment | SME worker RM200 first-year subsidy |
|---|---|---|
| Nature | Voluntary medical insurance/takaful | Government support towards eligible workers' first-year premium |
| Intended users | Eligible members of the public on rollout | Workers linked to qualifying SMEs |
| Must the employer apply? | Employer is not the only route to this public product | Exact subsidy claim channel not yet published |
| Amount | Actual final premiums depend on published product terms | RM200 per qualifying worker for first year |
| Is it cash in the worker's bank account? | Not applicable | Not announced as a cash handout |
| Timing | Planned January 2027 nationwide rollout | Subsidy application start and procedure still pending |
BNM's Budget 2027 statement also states that RM40 million is allocated for MediAsas vouchers for employees of SMEs with fewer than 75 staff. This is designed to extend health protection, not to distribute cash freely.
Fewer than 75 employees vs maximum 50 subsidised workers
These numbers mean different things.
- Fewer than 75 workers is the announced size threshold for the qualifying SME. For example, 74 is below 75; 75 is not.
- Maximum 50 per company caps how many workers can be supported under the proposed subsidy. An SME with 60 employees does not automatically receive 60 subsidised premiums.
| Illustrative company workforce | Under 75? | Maximum calculated support at RM200 each |
|---|---|---|
| 10 | Yes, subject to other formal conditions | RM2,000 |
| 30 | Yes | RM6,000 |
| 50 | Yes | RM10,000 |
| 60 | Yes | Up to RM10,000, not RM12,000 |
| 75 | No under this particular size criterion | Do not assume eligibility |
These are mathematical subsidy examples, not an approved company grant or unrestricted cash payment. Final employee eligibility and allocation mechanics may affect actual amounts.
What should employees ask HR?
A practical, low-pressure question is:
"Will our company consider participating in the 2027 MediAsas SME employee premium subsidy? Is there any official HR process for the RM200 first-year support? If I prefer personal coverage, which participating insurer should I contact after launch?"
Even if HR does not know yet, the discussion can clarify whether the company is monitoring the programme. Do not tell staff that employers are already legally required to apply on their behalf; that has not been established.
Can I pay MediAsas with EPF Akaun Sejahtera?
Budget 2027 announces that EPF members below 55 will be allowed to use Akaun Sejahtera to pay MediAsas premiums. This is payment from your own EPF savings, not another government RM200 cash benefit.
Before choosing that approach, consider whether the coverage fits your needs, what its renewals cost, the long-term impact on retirement savings and what official payment method will apply.
An employee should not infer that the RM200 support eliminates all remaining insurance expenses.
What does MediAsas cover and is the premium already fixed?
BNM's MediAsas overview says it is a standalone, voluntary basic MHIT plan designed around more standardised benefits. It is expected to include aspects of pre-hospitalisation, hospitalisation, post-hospitalisation and certain outpatient care, with cost-sharing features.
BNM states a target indicative monthly premium range of RM60–RM550 for specified entry ages. Those are not the final January 2027 quoted premiums. Check final benefit limits, exclusions, waiting periods, co-payments, panel hospitals, underwriting and renewal arrangements when the plan launches.
The 2026 pilot involves selected participants, insurance/takaful operators and hospitals, rather than a fully open sale to the entire public.
How is it different from an existing medical card?
Existing health insurance policies vary in annual limits, panel hospitals, room-and-board arrangements, exclusions, additional benefits and premium increases. MediAsas is designed as a more standardised essential option, not a guaranteed superior replacement for every policy.
Compare actual benefits, out-of-pocket costs, deductibles, future premiums and suitability, not just a RM200 one-off incentive.
For the wider context of repricing, see JomGuide's previous article on Medical Card Premium Increases and MediAsas. This article instead focuses on personal enrolment versus an employer-related first-year subsidy.
What can an employee do now?
- Check the official BNM MediAsas page for the planned January 2027 rollout.
- Ask HR if your employer potentially satisfies the fewer-than-75-workers threshold and plans to monitor the subsidy.
- For individual enrolment, contact participating insurers/takaful operators after official nationwide launch rather than relying on unverified advance links.
- If under 55, understand how your EPF Akaun Sejahtera works before making a premium-payment decision.
- Do not send MyKad copies, EPF credentials or money to an unofficial site claiming immediate access to the RM200 scheme.
FAQ
Can non-SME workers buy MediAsas?
The plan is intended for public rollout and is not restricted solely to SME employees. Individual acceptance depends on final product terms.
Must SME staff apply through their boss?
The subsidy claim procedure has not yet been confirmed. That uncertainty should not be confused with ordinary public MediAsas enrolment.
Will a 10-person SME receive RM2,000 in cash?
Not as a confirmed free-cash grant. It is an illustrative cap on first-year premium support if all ten workers qualify.
Can a 60-worker SME get RM200 for all 60?
The announcement caps the benefit at 50 workers per company.
Does the RM200 repeat every year?
Only the first-year premium subsidy has been announced.
Is MediAsas automatically better than every existing card?
No. Compare final coverage, exclusions, costs and personal needs.
JomGuide's view
Workers should separate the right to purchase the public product, potential SME voucher eligibility, and the yet-to-be-confirmed subsidy application process. Do not cancel existing health protection or treat EPF savings as free money just to pursue the RM200 figure.
